Workplace Accommodations
UHD affords equal employment opportunity and provides reasonable workplace accommodations to qualified employees with disabilities as well as persons with sincerely held religious beliefs and women affected by pregnancy.
To request an accommodation, complete the Request for Workplace Accommodation Form. Medical accommodations also require a physician to complete the Medical Inquiry Form.
Submit completed forms to the ADA Coordinator located in HR. Supervisors that receive an accommodation request must immediately notify the ADA Coordinator.
