Eligibility
If you sustain an injury while employed at UHD, you are entitled to receive compensation under the State Employees' Workers' Compensation Program. In the case of a fatality, your legal beneficiaries are entitled to benefits.

Employee Responsibilities
All claims must be processed through the HR Benefits Department. To establish a claim for compensation as an injured employee, you must notify the university and complete all necessary forms. To start, fill out the Employee's Report of Incident or Injury form within 24 hours of the incident or injury:
Employee's Report of Incident or Injury
An incident is any unplanned event that does not result in personal injury or damage to property; however, given different circumstances could have resulted in a loss.
An injury is any unplanned event that does result in personal injury (including death) or damage to property.
You will need to attach the following to the Report of Incident or Injury form:
Network Providers
Texas Insurance Code Chapter 1305 authorizes the establishment of workers' compensation health care networks. The State Office of Risk Management (SORM) has contracted with the CareWorks Texas HCN Network to assist employees in the selection of network providers.
With the exception of emergency care, if you live within the CareWorks CompKey Plus Network coverage area, you are required to obtain medical care from a network provider to receive workers' compensation benefits.
