Family Medical Leave/Parental Leave - Employee Process Guide
Medical Leave
Employee notifies Supervisor at least 30 days prior to leave, or as soon as possible if emergency
Employee completes Family Medical Leave or Parental Leave Application
Employee submits Application to HR Benefits 1 month prior to leave, or within 3 days if emergency
HR Benefits completes employer section of Health Provider Certification and submits to Employee
Employee secures completed certification from Health Care Provider within 15 days of receipt and returns to HR Benefits
HR Benefits reviews Application. If approved, HR Benefits provides copies of the Application to Employee and Supervisor. If denied, Employee notified
If Health Provider Certification is unclear, HR Benefits will contact Employee
Supervisor enters and approves Absence Management Request(s) in TRAM for approved time
Employee will pay ERS any insurance premiums due while on unpaid leave for an entire month
HR Benefits will collect any missing premiums upon return from leave
Employee returns to work and provides release to return to work from Health Care Provider, if employee health condition

Medical Leave includes serious health condition of the employee; birth of a child; serious health condition of a child, parent or spouse of the employee; injury or illness of a service member who is a spouse, child, parent or next of kin of the employee.
Employee must use sick and vacation leave accrued; Employee will be placed on leave without pay when accrued leave is exhausted.
Non-Medical Leave
Employee notifies Supervisor at least 30 days prior to leave, or as soon as possible if emergency
Employee completes Family Medical Leave or Parental Leave Application and attaches applicable documentation
Employee submits Application to HR Benefits 1 month prior to leave, or within 3 days if emergency
HR Benefits completes and approves Application and provides copies of the Application to Employee and Supervisor
Supervisor enters and approves Absence Management Request(s) in TRAM for approved time
Employee will pay ERS any insurance premiums due while on unpaid leave for an entire month
HR Benefits will collect any missing premiums upon return from leave
Employee returns to work as scheduled

Non-Medical Leave includes birth and care of a child; placement of a child for adoption or foster care; a qualifying exigency for a spouse, child, or parent a service member who is on active duty or notified of any impending call to active duty.
Employee must use vacation leave accrued; Employee will be placed on leave without pay when accrued leave is exhausted.
